Passport Application Instructions
As of November 1, 2016 eye glasses are
no longer allowed in passport photos.

All passport applications require 1 photo (2” x 2”/ 5cm x 5cm) in color with a white background and no eye glasses

Renew a Passport
Adult Renewal
If your most recent passport: was issued after your 16th birthday and was issued less than 15 years ago; is currently in your possession and has the same name you currently use
- Applicant must be present at the Embassy
- Applicant’s most recent passport
- Completed and signed form DS-82 (PDF 84K)
- Fee of $110 payable in either ariary or dollars (cash or Visa card are accepted)
If most recent passport: is lost or stolen OR was issued before 16th birthday
- Applicant must be present at the Embassy
- Applicant’s most recent passport (if it’s in your possession)
- Birth certificate/CRBA or other proof of identity and citizenship (your passport is not in your possession)
- Fee of $145 payable in either ariary or dollars (cash or Visa card are accepted)
- Completed form DS-11 (PDF 97K) (do not sign until you are in front of a consular officer)
- Completed form DS-64 (PDF 51K) (if your most recent passport was lost or stolen)

Minor Applicants
Minor Passport Issuance and Renewal (for children under the age of 16)
- Both parents and the child must be present at the Embassy (if one parent absolutely cannot be present, form DS-3053 must be submitted)
- Child’s birth certificate/CRBA
- Both parents’ passports
- Photos of the child growing up
- Fee of $105 payable in either ariary or dollars (cash or Visa card are accepted)
- Completed form DS-11 (do not sign until you are requested to do so by a consular officer)
